Disease Management and Health Monitoring for ACOs

Research and Markets has announced the addition of the "Disease Management and Health Monitoring for ACOs" report to their offering.

This report examines new care delivery models such as patient-centered medical home (PCMH) and Accountable Care Organization (ACO).
